What is an MLTC and how does it apply to the applicant?

Today, all Medicaid applicants requiring home-care services must join a Mange Long Term Care plan (“MLTC”). There are several MLTC’s to choose from, but basically all offer the same services with very little differences. Once the applicant is financially approved by Medicaid the applicant needs to get a medical assessment by a Medicaid nurse. Upon completion of the Medicaid nurses’ assessment the nurse will instruct the applicant to call the MLTC of their choice and arrange for a nurse to assess the applicant for the hours of home-care services.

The MLTC will provide not only the home care services, but will assign a coordinator for all of the patient’s medical needs and issues. At times the Coordinators are the patient’s nurses, which makes life a little easier since the nurse personally knows the patient.

Managed Long Term Care (MLTC), is a process which offers long term services such as home care to the elderly and disabled who wish to live in the community in their homes.

Once the applicant is approved for Medicaid they choose an MLTC of their choice from a list of Medicaid approved agencies who offer home care services and many other services listed bellow:

SERVICES PROVIDED BY THE MLTC:

Home Care:
Home Attendant, Physical Therapy, Occupational Therapy, Speech Pathology and Medical Social Services.

Adult Day Care; Personal Care, including some surgical supplies, hearing aid batteries, prosthetics, orthotics and orthopedic footwear.

Personal Emergency Response System (PERS)

Dentistry, Optometry and Eyeglasses.

Consumer Direct Personal Assistance Program (CDPAP)

CDPAP allows the applicant’s representative to hire anyone, including but not limited to family members, to service the applicant. CDPAP offers the ability to care for the applicant with some of the more difficult tasks which home attendants are not allowed to undertake.

It’s advisable to seek a professional to assist applicants with filing their Medicaid applications and assist them with the Medicaid Enrollment Center as well as choosing the proper MLTC.
